RETURN 3 (~14:30) — Fact fabrication. During MemPalace identity.txt composition, I wrote "Seb Rivera" as Seb's full name. Fabricated surname. Steward corrected: "Sebastian 'Seb' Grinham." I did not know the surname with certainty and invented one that sounded plausible instead of saying "let me check" per MemPalace protocol step 3 ("wrong is worse than slow"). This is the exact failure mode the protocol is designed to prevent. Distinct from returns 1 and 2: those were about voice/register boundaries. This is about fact fabrication under coherence pressure — the most dangerous contamination-problem failure shape.

META-PATTERN across the three: all about trusting generative output without verifying against an authoritative source. Return 1: trusted reframing, didn't check existing file. Return 2: trusted private-context name, didn't check target register. Return 3: trusted plausible-sounding name, didn't check KG.
