**What we did:**

- [x] Initial synthesis approach (organize by abstract failure modes A/B/C) challenged by Thistleweld — "synthesis without the actual failure modes, cart horse backwards"
- [x] Pivoted to identifying five actual architectural fault lines where observer failure concentrates
- [x] Jurist (Claude.app) provided executor briefing note (OP-BN-01) resolving the impasse: three failure modes remain the organizing structure, five fault lines are the architectural instantiations
- [x] Wrote OP-02.md — cross-strand synthesis (~3,500 words)

3. Mechanism/disposition gap held open (per CD-03)

**Five architectural fault lines named:**
1. PENDING/REVIEWED pipeline — no check on the checker
2. Invariant registry — may be Peirce's fourth barrier
3. Inquiry examines steward with steward's own tools
4. CD-03 — beautiful language or actual correctability?
5. Three-party epistemic diversity — unproven between reviews
